SAN BERNARDINO (KTLA) -- Investigators believe that the driver of a church bus that crashed in the San Bernardino Mountains may have drifted into oncoming traffic.

A CHP report released Tuesday afternoon stated that Won Seok Chae, 61, drove left of the double yellow lines and into the opposing lane but are still unsure why.

Chae was the only person killed in Monday's crash on Highway 189.

Chae was driving the bus carrying 23 passengers -- most of them teenagers -- when he lost control around a curve about half a mile north of Lake Gregory.

Officials say the bus collided head-on with a Nissan Murano that belonged to the San Bernardino County Fire Department, before slamming into a tree and knocking down a power pole.

The bus then slid about 20 feet off the side the roadway down a snow covered embankment, according to California Highway Patrol.

Chae and the passengers were all members of the "Light of Love Mission Church" in Pasadena and were returning home from a church retreat in Twin Peaks about 10 miles from Lake Arrowhead.

Investigators say there were no seat belts on the bus.

Multiple ambulances were on scene and a triage was set up by firefighters on the side of the road.

A San Bernardino County Fire Battalion Chief and his wife were inside the SUV that collided with the bus. Both were rescued from the vehicle.

The man was reported to have sustained minor injuries, and the woman was extricated with moderate injuries, officials said.

The injured were taken to Loma Linda Medical Center and Arrowhead Regional Medical Center. One victim, a 12-year-old girl remains in critical condition Tuesday.

The 12-year-old girl is in "extremely critical condition" according to doctors. She suffered a head injury and is unconscious.

A 17-year-old boy and a 28-year-old woman also remain in critical condition.

Nine crash victims arrived at Arrowhead Regional Medical Center following the crash. Four were discharged and two patients with skeletal bone fractures -- listed in "very stable" condition and were transferred to Kaiser Permanente in West Los Angeles.

7 patients, 3 in serious, 4 in fair condition, remain hospitalized at Loma Linda University Medical Center.

Members of the Light of Love Church held a vigil Monday evening to pray for the injured.

Deacon John Cho said the church has been in existence for 18 years and has about 1,000 members.
